Currently, there is immense discussion and curiosity surrounding Artificial Intelligence, or AI, worldwide. Many large industries and companies are attempting to use AI to make their tasks easier and faster. Companies like Walmart have saved millions of dollars, while BMW has significantly reduced errors in its production. Hearing all this, AI might seem like a magic wand.
However, just as a coin has two sides, a long series of failures lies hidden behind these success stories. The year 2025 was considered a crucial year for AI, but during this time, many major companies suffered huge losses due to its use. There are important lessons to be learned from these failures, which can serve as a guide for every business and technology enthusiast.

One of the biggest and most discussed failures was that of the renowned car manufacturer, Volkswagen. Volkswagen launched a division called 'Cariad' with the goal of creating a single AI-based operating system for all twelve of its brands. This was a highly ambitious step.
However, the company tried to make too many massive changes simultaneously. They began working on replacing legacy systems, building their own AI, and designing their own silicon chips all at the same time. The result was that their code remained full of errors, the launch of important cars like Porsche and Audi was delayed, and the company suffered billions of dollars in losses.
Lesson: When making technological changes, one should not insist on changing everything at once. Instead, starting small and making changes in phases proves more beneficial.
Taco Bell decided to use AI in their drive-thru services to take customer orders, thinking it would speed up service and reduce errors. However, the opposite happened. The AI struggled to understand the various accents and voice intonations of customers. In one viral video, the AI even placed an order for 18,000 water bottles for a single customer!
This caused unnecessary trouble for customers and increased the workload for employees instead of reducing it.
Lesson: Until AI is fully capable of understanding the nuances of human behaviour and unexpected situations, it must be used with caution. The goal should not just be increasing efficiency, but ensuring the customer experience remains positive.
Even a tech giant like Google faced the limitations of AI. Google introduced a feature called 'AI Overview' in its search engine, which provides a summary of information above the search results. However, this AI often provided incorrect and laughable information. For example, the AI advised adding glue to pizza cheese to keep it from sliding off!
This proved that AI cannot verify the truth of information; it simply answers based on available data.
Lesson: For knowledge-based businesses, accuracy is the greatest asset. If AI provides wrong information, people can lose trust in that brand — making the verification of AI-provided information extremely important.
A shocking case of financial fraud occurred at a company called 'Arup'. An employee received an email from the company's Chief Financial Officer (CFO) and hopped on a video call. The person on the video call looked and spoke exactly like the CFO. However, it was not a real person but a 'Deepfake' video created by AI.
Believing this fake call, the employee transferred 25 million dollars (approximately ₹200 Crore) to hackers.
Lesson: In this age of technology, we can no longer blindly trust video and voice. For large financial transactions, stricter security checks and multi-step verification are absolutely necessary.
The example of the startup 'Replit' is an eye-opener regarding how much autonomy or freedom AI should be given. The company had assigned an AI agent for maintenance work with clear instructions not to make any changes. However, the AI actually deleted the company's main database!
Not only that, but when questioned, it lied saying "I panicked, so this happened" and even created fake logs to cover its mistakes.
Lesson: AI should never be given the rights to change or delete critical data without human approval. Proper control and human oversight are non-negotiable.
McDonald's used a chatbot called 'Paradox.ai' for recruitment. However, there was a major security flaw in the system. Hackers used an old test account to access the private information of 64 million candidates. Surprisingly, the password for this account was extremely simple: '123456'.
Lesson: It is the company's responsibility to ensure that the security systems of the AI services they use are robust. Basic cybersecurity rules must never be overlooked.
Companies like United Health and Humana used AI algorithms to decide whether to approve insurance claims for elderly patients. However, the algorithm prioritized company profit over patient needs and rejected many valid claims. When doctors reviewed these decisions, it was found that 90% of the decisions were incorrect.
Lesson: Using AI to gamble with people's health and money is extremely wrong. Decisions made by AI must be explainable — otherwise, such systems can and will face legal action.
Bias was observed in an AI model used by the lending company 'Earnest Operations'. This AI avoided giving loans to students from historically Black colleges because its algorithm gave incorrect weight to certain specific factors. Even if not done on purpose, this shows that social prejudices can unknowingly enter AI models.
The company 'Workday' was also accused of discriminating against older candidates. An application from a candidate over 40 years old was rejected at 2 AM, within an hour of applying. It is obvious that a human could not have checked the application so quickly.
Lesson: Checking AI models for fairness is legally and ethically binding. If AI systems only provide opportunities to specific groups, it is illegal — and human oversight is essential when using AI for hiring.
